Instructions: Complete the evaluation when the program is complete and/or the grant funding has been fully expended. The form is completed and submitted on the website. You may “Save and Continue Later” – the site will email you a link which is active for 30 days.

Evaluation responses should include references and outcomes as connected to your original application.

Evaluation Narrative

Program / Project narrative responses should include information about the specific grant-funded project and impact on the organization's overall work.

Capacity Building/Organization Support grants should include information and impact on the organization's ability to serve its community and how the grant assisted in the organization's work within the community.

Measurable Impact within the Organization

What has been the project's measurable impact to date on your organization? Has the grant (1) helped to attract new funding from the community, (2) increased collaboration with other organizations, (3) increased volunteer involvement? How has your organization changed as a result of this project or support? (Maximum of 3,500 characters with spaces)

Adjustments

We recognize that circumstances can change, either from the time of the application submission or during the implementation process, possibly affecting the implementation of the grant. What, if any, difficulties or challenges have you encountered; and what refinements or plans have been made to overcome them? (Maximum of 1,500 characters with spaces)

Overall Success

If you feel your grant-supported need has been successful, what factors contributed to this? If the project or grant-supported need was less successful than you hoped, what do you think contributed to that? What have been the most challenging or surprising aspects of this project? Have there been any unexpected outcomes? (Maximum of 2,000 characters with spaces)

Post-grant plans

Evaluation at its best is a tool for learning. How will your organization use what you have learned from this grant-funded project or need to guide your planning and activities in the future? If this grant supported a specific project, will it be continued? If so, how will you sustain it? (Maximum of 1,500 characters with spaces)
